Required Forms and PaperWORK

Click to the buttons on the left to download and print all of the required paperwork for 7th-12th Grade band members.  All of these forms must be on file with the band office before a student can participate in any extra curricular band function.  Medical Release Forms only need to be filled out once, unless they require updating.  Permission Slip and Handbook Acknowledgement Form need to be filled out annually.
